Veteran Education Benefit Forms
Veterans Intake Form
This form needs to be completed by any student who has never used Veteran Education Benefits at WKCTC or any student who hasn’t used them in more than 2 years. The information is used to verify to the VA the correct address, phone number, etc. for the student.
Military Transfer Credit Form
This form needs to be completed by Veteran’s who are submitting their JST (Joint Services Transcript) for evaluation. In addition to the form, WKCTC needs the student to request an official JST to be sent to us. Contact us if you have trouble with this.
VA Benefit Certification
This form must be completed each semester that a student plans to use VA education benefits.
Informed Statement
This form needs to be read and signed off by each student using benefits. If you are using CH 30 or 35, complete that form. If you are using VA Vocational Rehabilitation, use the CH 31 form. If you are using Post 9/11 GI Bill®, use the CH 33 form.
